  • Teaching Tuesday!! Today we are going to briefly talk about ultrasound, specifically the FAST exam. Here we are teaching the F.A.S.T. exam to several flight nurses. The F.A.S.T. exam, or the Focused Assessment with Sonography for Trauma exam, is an important test when it comes to trauma patients. The basic FAST exam evaluates 4 areas on the body: the perihepatic space or Morison's pouch, perisplenic space, pericardium, and the pelvis. We are trying to see fluid, likely blood in a trauma, which can then help surgeons and emergency physician plan the next steps for the patient. A positive scan showing fluid in abdomen, in an unstable patient it would likely lead to the patient going immediately to surgery for an exploratory laparotomy. If the patient is stable enough, they may go to get a CT scan of the abdomen to more specifically locate the area of injury. If there is blood in the pericardium, or around the heart, the patient may receive a pericardiocentesis or thoracotomy to remove the fluid from around the heart to prevent a cardiac arrest. . . The FAST scan is sensitive but not specific. It can tell you if there is blood in the abdomen but likely cannot tell you where it is coming from. That is why a CT scan is needed, or if the patient is unstable, the surgeons will open the abdomen and search meticulously for the source of bleeding. This exam was probably my favorite part of being a medical student in the emergency department. You are vital to the patient's care, it is easy to learn, and you are right in the middle of the trauma next to all the hot shot doctors. Clearly he was not & 2 days later he was admitted to the hospital. Eliseo has been hospitalized on multiple occasions due to the non compliance w/Drs Treatment plan. If his treatment plan is not followed & he does not get proper medical treatment he will die. He is 12 going to school soiled w/ pee& poop & his mother & her family DO NOT CARE!!! THEY TELL HIM THAT HE IS NOT ALLOWED 2 TAKE HIS MEDS & THAT HE DOESN'T NEED IT EVERYTIME HE ASKS FOR THEM. how does that make any sense when it was PRESCRIBED by a Doctor!! They are KILLING HIM SLOWLY. The Doctors & Hospitals have called CPS multiple time due 2 multiple levels of Neglect & yet nothing has been done. The court system is NOT HELPING THIS BOY!! A single sneeze can release up to 40,000 droplets.Each one of these droplets may transmit the disease, since the infectious dose of tuberculosis is very small (the inhalation of fewer than 10 bacteria may cause an infection). People with prolonged, frequent, or close contact with people with TB are at particularly high risk of becoming infected, with an estimated 22% infection rate.A person with active but untreated tuberculosis may infect 10–15 (or more) other people per year.Transmission should occur from only people with active TB – those with latent infection are not thought to be contagious.The probability of transmission from one person to another depends upon several factors, including the number of infectious droplets expelled by the carrier, the effectiveness of ventilation, the duration of exposure, the virulence of the M. tuberculosis strain, the level of immunity in the uninfected person, and others.The cascade of person-to-person spread can be circumvented by segregating those with active ("overt") TB and putting them on anti-TB drug regimens.   • Jeg er læge! One small step for mankind, one giant leap for me🙏🏻 Var til sidste eksamen i torsdags og fik den endelige karakter i dag. Det har været en vanvittig rejse, en rejse fyldt med kæmpe forhindringer der til tider slog mig så hårdt i gulvet, at jeg ikke fatter jeg kom op igen. Et år inde i studiet – blev jeg diagnosticeret med dysleksi (ordblindhed). En meget sen alder, at finde ud af det og en lille del af mig vil nok altid tænke, hvor anderledes mit liv havde set ud, hvis der var nogen der havde set det da jeg var barn. Mange unødvendige nederlag jeg havde kunne undgå. Derfor er jeg også meget ærlig omkring det, for hvis min åbenhed kan hjælpe bare én, så er det ‘det værd. Dysleksien betød jeg var 4 år om en bachelor der tager 3 år, til gengæld gennemførte jeg kandidaten på 2,5 år, i stedet for 3 år (plus lidt forskning til mit CV) Så ja – det er meget udansk at sige, men jeg er ret stolt af mig selv😊 Sejren er ikke min alene, mange har kæmpet med og For mig. Så kæmpe tak til @linegronbaek for så velvilligt, at dele dine noter og altid samle mig op når jeg bukkede under for presset. Jeg havde aldrig klaret den uden dig. Tusind tak til henholdsvis Kristian Kraft Hansen og @mathildeec for at være mine fantastiske tutorer, der hjalp mig med at lære at læse på ny og give mig selvtilliden tilbage. Tak til mine forældre der tog min hund i pleje, så jeg kunne pendle mellem København og Odense og for utallige gange, at stå op før daggry for at køre mig til toget, når jeg valgte at dele turen op i to og overnattede hos jer.   • Does rectal fluid contain HIV? In an HIV-positive person, the mucous membranes throughout the body can contain a lot of HIV. This is because these membranes are rich in immune cells, which are the cells that HIV likes to infect and replicate within. Since so much HIV replication can occur at the mucous membranes, the virus is able to enter the mucus that the membranes produce. The mucous membranes of the rectum, and the mucus they produce (rectal fluid), are no exception. Several studies show that HIV can be found in the rectal fluid of a person living with HIV. In fact, one study of 64 HIV-positive men (of which about half were on antiretroviral therapy) found that, overall, the average amount of virus in their rectal fluid was higher than in their semen and blood. Why might rectal fluid contain more HIV than other bodily fluids? It turns out that the majority of the immune cells in the body including the cells that are a major target for HIV are located in the mucous membranes of the gastrointestinal tract, which includes the rectum. There are a lot of immune cells in the gastrointestinal tract because it has a very large surface area. Also, a large number of immune cells are needed to help to protect the gut from the “foreign” germs in our food and to control the growth of the “friendly” germs living in our gut. The high concentration of immune cells means that the majority of HIV replication in someone with HIV may be happening in the gastrointestinal tract, including the rectum. This may explain why so much HIV can be found in the rectal fluid. Rectal fluid has implications for HIV transmission through anal sex when the HIV-negative person is the insertive partner (that is, inserts their penis into a partner’s anus). Research show that this type of anal sex can carry a significant risk of HIV transmission. Rectal fluid undoubtedly contributes to the risk of HIV transmission through anal sex where the insertive partner is HIV negative.

Se trata de una situación que implica graves riesgos. Por un lado, los infectados no diagnosticados tienen grandes posibilidades de convertirse en transmisores del VIH. La Plataforma VIH en España estima que la tasa de transmisión es 3,5 veces mayor entre los que desconocen su estado seropositivo que entre los que ya cuentan con un diagnóstico. Y algunos estudios que la Plataforma menciona indican que el 25% de los infectados que desconoce su estado es responsable del 54% de las nuevas infecciones. Por otro lado, a nivel individual, el diagnóstico tardío aumenta la mortalidad de los pacientes. Un dictamen a tiempo es lo que ha permitido que el sida se haya convertido en una enfermedad crónica con una expectativa de vida potencialmente normal, gracias a las terapias existentes en España, donde cualquier persona diagnosticada de VIH tiene derecho a tratamiento gratuito Desde el punto de vista económico, varios estudios indican que el coste de tratar a los pacientes que presentan un retraso diagnóstico puede ser un 200% más caro que el de un paciente diagnosticado a tiempo.
